ALL TEAM CHECK IN WILL BE COMPLETED ONLINE AND THROUGH GOTSPORT, FREE OF CHARGE.

You must upload the following items to your team gotsoccer account (link at the bottom of this page)

1. Your current 2023-24 approved roster with all jersey numbers.
2. USYS, US Club, AYSO, SAY or USSSA player and coach cards/passes for all participating players (including guest players) in one file and in order they are listed on your roster.
3. Permission to Travel Form for all USYS teams registered outside of the State of Illinois.
4. Guest Player Forms. Players within the same club are not considered Guest Players and can be written on the roster with player name, ID number, birth date and jersey number.

**PLEASE SEND ALL DOCUMENTS IN A PDF FORMAT AND IN ALPHABETICAL ORDER. DOCUMENTS SHOULD BE IN 1 FILE FOR EACH SPECIFIC CATEGORY.

Guest players from a different club will each require a Guest Player Form ( acquired from the organization with whom you register) to be uploaded.  Guest players should be written on roster with ID numbers, birth date & Jersey number. You may have up to 5 guest players.

*22 player on 13U - and older teams ( all 22 can play in each game)

*16 players on 12U and 11U 9v9 Rosters 

*14 players on 10U and younger 7V7 rosters

Team Instructions:

***All players, including guest players, must be listed on the teams "IYSA Sanctioned Tournament Roster or their respective League Roster."

***Each team must submit the "IYSA Tournament Risk Management Certification" per IYSA sanctioning laws.  If you need a "Youth Sports Completion Certificate,"  you can click the link below.

Illinois Youth Soccer Association IYSA - affiliated teams (IWSL, YSSL, CIYSL, SISL, IllOWA) submit:
1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site, the IYSA site, or your team page in your league’s site). All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster or League roster.

2. IYSA Medical Release forms for all players (Not Needed for USYS Teams) 

US Club teams (NISL, out-of-state US Club teams) submit:
1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site or the IYSA site). All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.
2. A copy of your US Club certified roster (NISL, …)
3. IYSA Medical Release forms for all players (US Club Medical Release forms cannot be accepted)
4. Form NT

Out of State USYSA affiliated teams submit: Kansas Youth Soccer, Wisconsin Youth Soccer, Michigan Youth Soccer, Minnesota Youth Soccer, Ohio South Youth Soccer, Indiana Soccer Association & all other out of state associations:
1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site or the IYSA site). All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.
2. A copy of your State certified roster (from the league you are playing in).
3. IYSA Medical Release forms for all players (out-of-state Medical Release forms cannot be accepted)
4. An approved “Permission to Travel” form from your home state’s USYSA affiliate.

Guest Players:  

• A team can have up to 5 guest players.
• All players on a team must have player passes from the same soccer affiliation. IYSA affiliated leagues include IWSL, YSSL, CIYSL, SISL, and ILLOWA. US Club affiliated local leagues include NISL and ICL. IYSA and US Club are not affiliated.

A player with a player pass from the same club and the same soccer affiliation is not considered a guest player.
